How to Diagnose Rotator Cuff Tears

Shoulder pain that suddenly makes it hard to lift your arm, reach behind your back or sleep on one side is rarely something to ignore. If you are wondering how to diagnose rotator cuff tears, the key point is this: diagnosis is not based on one symptom alone. It comes from the pattern of pain, weakness, movement loss, clinical testing and, where needed, imaging to confirm exactly which tendon is involved and how severe the injury is.

The rotator cuff is a group of four tendons and muscles that stabilise the shoulder and help with lifting and rotation. Tears can happen after a fall, while lifting something heavy, or more gradually through wear and degeneration over time. Some tears are small and painful but still manageable without surgery. Others cause marked weakness and loss of function and need urgent specialist assessment. Getting the diagnosis right early matters because treatment decisions depend on the size of the tear, the tendon affected, your age and to some degree your management preference.

How to diagnose rotator cuff tears in clinic

A proper diagnosis starts with the history. That sounds simple, but it is often the part that gives the strongest clues. A sudden tearing sensation after a fall onto the arm points to a different problem from shoulder pain that has been building for months with no clear injury. Night pain, pain when reaching overhead, difficulty putting on a coat and weakness when lifting away from the body are all common features of a rotator cuff problem.

Your clinician will usually ask when the pain started, whether there was trauma, where the pain is felt, and whether the problem is mainly pain, weakness or stiffness. That distinction matters. Rotator cuff tears often cause both pain and weakness. Frozen shoulder, by contrast, tends to cause more global stiffness. Arthritis may cause pain and restriction, but not the same pattern of tendon weakness.

The physical examination is the next step. This looks at range of movement, strength and pain provocation. A clinician may ask you to lift the arm forwards, out to the side and rotate it inwards and outwards. If movement is full but painful, that suggests one pattern. If you physically cannot generate strength in a particular direction, that raises suspicion of a more significant tear.

There is no single perfect test. Instead, diagnosis relies on a combination of findings. Painful weakness with resisted abduction may suggest supraspinatus tendon involvement. Weakness in external rotation can point towards infraspinatus pathology. Difficulty with internal rotation or lift-off type movements may suggest subscapularis injury. These tests are useful, but they are not enough on their own. Pain can inhibit strength even when a tendon is not fully torn.

Symptoms that raise suspicion of a tear

Rotator cuff pain is often felt over the outer shoulder and upper arm. It can be worse at night, particularly when lying on the affected side. Many patients describe a painful arc when lifting the arm, with symptoms worsening somewhere between waist level and shoulder height.

What tends to raise concern for a tear rather than simple irritation is weakness. That may mean struggling to lift a kettle, place items in a cupboard or control the arm when lowering it down. After a traumatic injury, a sudden inability to raise the arm should be taken seriously. In an older adult, a degenerative tear may present more gradually, sometimes after a relatively minor event that simply exposes an underlying tendon problem.

It is also worth noting that not every tear causes dramatic symptoms. Some partial tears behave like tendinopathy and mainly produce pain. Some full-thickness tears are found on scans in people who have no symptoms what so ever. That is why scans must always be interpreted in the context of symptoms and examination, not in isolation.

Clinical tests and what they can tell you

If you want to understand how to diagnose rotator cuff tears accurately, clinical examination is where suspicion becomes more focused. Certain tests are designed to load individual tendons or reproduce the mechanics that trigger symptoms.

For the supraspinatus, clinicians often assess pain and weakness with the arm elevated in a controlled position. For infraspinatus, resisted external rotation is commonly tested. For subscapularis, internal rotation strength and specific manoeuvres that bring the hand behind the back may be used. The pattern matters more than any one positive result.

A good shoulder assessment also checks for other causes of pain. Neck-related pain can refer into the shoulder. AC joint problems may cause pain over the top of the shoulder. Biceps tendon issues can sit alongside rotator cuff pathology and sometimes confuse the picture. If the shoulder is very stiff in all directions, frozen shoulder may be the dominant issue rather than a tendon tear.

This is where specialist assessment adds value. The goal is not just to say there is a tear. The goal is to identify which structure is involved, whether the tear is partial or full thickness, whether there is associated bursitis or biceps pathology, and what that means for treatment now rather than after a long delay.

Do you need a scan?

Often, yes – but not always immediately. Mild shoulder pain with preserved strength may first be managed conservatively if the history and examination suggest tendon irritation rather than a significant tear. On the other hand, if there has been trauma, marked weakness, persistent night pain or failure to improve, imaging becomes much more useful.

Ultrasound is an excellent first-line investigation for many rotator cuff problems. It can show partial-thickness tears, full-thickness tears, bursitis, calcification and dynamic tendon movement in real time. It is quick, accessible and particularly helpful when combined with expert clinical examination. For many patients, that means faster clarity and a more direct treatment plan.

MRI is sometimes preferred if there is concern about more complex injury or labral pathology in traumatic cases. It gives a broader picture of the joint and surrounding soft tissues but is often not required for diagnosing rotator cuff tears.

X-rays do not diagnose rotator cuff tears directly, but they can help identify arthritis, calcific change or bony features that contribute to shoulder pain. In some cases, they are used alongside ultrasound or MRI rather than instead of them.

Partial tear or full-thickness tear?

This distinction affects both urgency and management. A partial tear means some of the tendon fibres are damaged but not completely disrupted. These tears can be painful and limiting, but many respond well to guided rehabilitation, activity modification and, where appropriate, image-guided treatment.

A full-thickness tear means the tendon is torn through. Even then, management is not identical for every patient. Some full-thickness tears in older adults can still be managed non-operatively if strength and function are acceptable. A recent traumatic full-thickness tear in an active patient is different and may need early surgical opinion. Timing matters because delayed assessment can make some tears harder to repair.

That is why a fast, accurate diagnosis is valuable. It helps avoid both under-treatment and over-treatment.

You should arrange prompt review if shoulder pain starts after a fall or injury and you cannot lift the arm properly, if weakness is getting worse, if night pain is severe, or if symptoms are not improving with rest and simple measures. Where a significant traumatic tear is identified, especially with clear weakness and loss of function, early referral to a shoulder specialist may be the best course. The important thing is precision. A vague label of shoulder strain is rarely helpful when pain is affecting sleep, work, sport or daily tasks.
